A delayed abnormal result

You are the medical registrar. Helen Ward, 61, has returned after a chest radiograph report recommending further investigation was not acted on for six weeks. The delay is confirmed; its effect on outcome is unknown. Your consultant has arranged urgent respiratory review and CT. Explain the delay, apologise and agree the next steps. No cancer diagnosis has been established.
